    Download and fill out the DS-11 Form from www. travel.state.gov or get this form at Passport authorized Post Offices.

    Get your 2×2 Passport Picture. Walgreens & CVS have $2 off coupons on their websites. Most authorized Post Office Passport locations also offer this service.

    Take the following to an authorized Post Office Passport Location:

    Filled-Out DS-11 Form

    One of the Following: Birth Certificate, Consular Report of Birth Abroad, Certificate of Naturalization/Citizenship

    One of the Following: Valid Driver’s License (plus a second ID if issued in a different state than where you apply), Undamaged U.S. Passport (if issued less than 15 years ago), Certificate of Naturalization, Valid government ID (city, state, or federal), Valid Military ID

    $110 Application Fee for Passport Book** (must be cash or money order) and $25 Execution Fee

    **You only need a Passport Book, you do not need the Passport Card. The Passport Card is not valid for travel to Israel
